ATHLETIC ELIGIBILITY, FORMS, AND FEES

It is important for you and your student to have read the handbook and filled out all forms before participating in athletics.

To participate in sports at Classical Prep, student athletes must have completed all forms below. All sports physicals must be completed and dated after April 1, 2020 to participate in any sports.
